Verbal Consent Script

By agreeing, you consent to receive text message alerts from Unity Care AI regarding door alerts or related security notifications.
These messages will be sent from our toll-free number and may include details such as the specific door and the time of the alert.
Message frequency depends on alert activity. Standard message and data rates may apply.
You can opt out at any time by replying STOP. Reply HELP for help.
Do you agree to receive these SMS alerts?”
Consent Collection Process
- This script is read aloud to the consumer at onboarding.
- Upon verbal agreement, the consent is logged immediately by the representative in our internal system.
Recorded Details
For each customer who consents, we log:
- Customer Name or ID
- Date and Time of consent
- Agent’s Name or ID
- Consent Status (e.g., Yes/No)
