What Is RPM/RTM?
Remote Patient Monitoring (RPM)
Remote Patient Monitoring enables healthcare providers to collect and assess physiological data from patients in real-time or near-real-time—without requiring them to be physically present. RPM is ideal for managing chronic conditions, monitoring post-discharge patients, and improving preventive care.
Examples of tracked vitals:
- Blood pressure
- Weight
- Heart rate
- Blood glucose
- Pulse oximetry (SpO₂)
RPM improves patient engagement, reduces avoidable hospital visits, and qualifies for Medicare/Medicaid reimbursement under CPT codes.
Remote Therapeutic Monitoring (RTM)
RTM expands remote care beyond vitals by focusing on patient therapy adherence, symptom reporting, and behavioral health trends. It’s used for:
- Musculoskeletal therapy (e.g. physical rehab)
- Respiratory monitoring (e.g. asthma, COPD)
- Mental health and behavioral interventions
RTM is reimbursable with its own CPT code set and supports value-based care goals by promoting early intervention and reducing ER visits.
RPM & RTM Workflow
Connect Devices
Patients are provided with FDA-cleared Bluetooth-enabled medical devices (e.g. A&D blood pressure monitors, smart weight scales, pulse oximeters). Devices are pre-configured and shipped directly or handed off at discharge
Data Transmission
Once used, data from the devices is automatically synced to our cloud platform using a secure gateway. This ensures consistent uploads with minimal patient interaction.
Review & Respond
Clinicians receive alerts, trend reports, and dashboards. You can also generate care summaries and patient messages as part of CMS engagement requirements.
Compliance & Billing
Our system tracks provider time, logs device usage, and generates billing-ready reports aligned with RPM/RTM CPT codes.

Supported CPT Codes
Remote Patient Monitoring (RPM)
- 99453 – One-time setup and patient education after 16 days of use in a 30-day period
- 99454 – Monthly device supply and data transmission for 16+ days of readings
- 99457 – First 20 minutes of clinical review and interactive communication per month
- 99458 – Each additional 20 minutes of RPM management (up to 60 minutes/month)
RPM services can be delivered under general supervision and billed by physicians or qualified healthcare providers. Only one provider may bill per patient per month.
Remote Therapeutic Monitoring (RTM)
- 98975 – One-time setup and patient education after 16 days of data in a 30-day period
- 98976 – Monthly supply of respiratory system monitoring devices (e.g., asthma, COPD)
- 98977 – Monthly supply of musculoskeletal system monitoring devices (e.g., rehab)
- 98978 – Monthly supply of cognitive behavioral therapy (CBT) monitoring devices
- 98980 – First 20 minutes of RTM data review and interactive communication per month
- 98981 – Each additional 20 minutes of RTM review and management
RTM is billed by qualified healthcare professionals (e.g., MDs, NPs, PTs, OTs). Interactive communication must occur each month, and documentation of time is required. 98978 is specifically used for CBT-related digital therapeutics.
